College of the Arts Vehicle Request Form

  • I acknowledge and attest that these travel plans will comply with USG Travel Regulations, and that I have completed:
    • A Travel Authorization Form (T-Auth) prior to the event (approved by supervisor);
    • The motor vehicle use video or completed the AAA Defensive Driving Course;
    • The drivers acknowledgement form;
    • The fuel card procedures/application and have a valid Fuel Card Driver ID/PIN.

    Rules and Regulations

    1. Only VSU employees are permitted to operate university vehicles. All vehicle drivers must have an active fuel card pin number, as well as have a valid driver’s license.

    2. Please review the VSU Motor Vehicle PolicyAll employees who drive their own car, a rental car or a state vehicle while on university business must have annual training. This training will allow you to complete the required Driver Acknowledgement Form. Additional requirements are needed for those employees who routinely drive state vehicles.

      IF THE COLLEGE OF ARTS’ VEHICLE IS INVOLVED IN AN ACCIDENT OR IN ANY UNLAWFUL INCIDENT, FILE A POLICE REPORT AND PROVIDE A COPY TO THE COLLEGE OF THE ARTS DEAN’S OFFICE IMMEDIATELY.
    4. If the requester needs the seats removed from the vehicle, please contact/inform the FAB Facility Coordinators; 229-333-5818. (Please allow at least 48hrs advanced notification for removal of seats)

    5. Seat belts must be worn at ALL TIMES by ALL OCCUPANTS while the vehicle is in motion.

    6. Before returning the vehicle, all trash and unnecessary debris must be removed. Failure to do so will result in a $100.00 fee charged to the requestor’s department.

    7. When returning the vehicle, submit a vehicle return form (Odometer reading, fuel level, indicate equipment issues and submit a report of any damage to the vechicle), and return the vehicle information packet/fuel card and vehicle keys to the COA Dean’s Office as soon as possible. Lost vehicle information packet, fuel card, or keys must be reported ASAP, and may result in loss of vehicle privileges. Lost vehicle keys will result in a $200.00 fee charged to the requestor’s department.

    8. College of the Arts vehicle requestors must return the vehicle to the Fine Arts Building Parking lot on the declared date of return with a full tank of gas – no matter the fuel level at the time of departure. Return all gas receipts to the requestors department for processing. USE REGULAR UNLEADED FUEL ONLY – NOT MID-GRADE OR PREMIUM GRADES.
