FLEX Roll-Over Form

Deadline for submission of rollover forms is seven (7) business days before the official graduation date for spring semester.

• Roll-over forms are accepted between March 1^st and the submission deadline [seven (7) business days before the official Spring Graduation date], but are not processed until after the semester is officially over.
• Roll-over forms do not need to be submitted in Summer or Fall terms.
• Forms MUST be submitted electronically below.
• If you have less than $25.00 in your FLEX it will automatically rollover.

I wish to roll my remaining FLEX account balance of $25.00 or more over until the end of the next Spring semester.  I understand that if my FLEX account remains inactive during said time, I shall forfeit the remaining balance and shall receive no prior notice.  I am aware that should I choose to withdraw my funds prior to the end of the next semester, I may complete one (1) Refund Request Form for a balance of $25 or more, 11 business days after the first day of each semester.
